While everyone involved is working very hard to overcome the challenges in healthcare, we know that the current system is severely over-stretched and under impossible pressure. Sláintecare is a cross-party, ten-year programme launched in 2019 by the Department of Health – and mapping out how our health and social care services can be transformed over the next 10 years, to better match our changing needs.

At the heart of Sláintecare is the ethic ‘right care, in the right place, at the right time’ – and one of the main drivers of the programme is ‘to provide the majority of care at, or closer to, the home’. This principle sits at the very heart of the work we do at Advanced Community Care.

We are firstly, totally committed to delivering ‘right care’. We put together care packages and support plans that are specifically tailored to each client’s individual requirements. We select and match our nursing staff with clients, and take time to get to know the families we work with, building up relationships of trust. It’s about putting together the exact right team, with the correct skills, knowledge and experience – and with the client at the epicentre. We also adapt the care we give as the client’s needs evolve, keep up with fast-moving technology and ensure our skills are constantly updated, through training and on-going education.

Making sure our nurses also get the ‘right care’ they need is a part of delivering the right solution too. As a nurse-led organisation, our nurses feel truly supported by Nurse Managers who understand their exact challenges and situations. They have walked in their shoes. They know what works and what doesn’t . Nurses feel ‘heard’, can access the advice, information or assistance they need at any time. The client is always paramount, and our focus is supporting staff to deliver on this. We are proof that teams who are well-valued and well-supported, always deliver a better service.

We believe we deliver to ‘right place’ as clients are able to be at home with their families and loved ones: always the first choice for care, if at all possible. At home, the risk of infection for clients is greatly reduced, while it also means less disruption for the families involved, avoiding the need for constant trips to and from hospital.

The personal, one-to-one nursing care we provide means the care is always given at the ‘right time’ – exactly when it’s needed. As nurses are working in this dedicated way, they feel present and totally focused on their patient’s needs… while all the essential equipment, medication and expertise is always immediately to hand.
