What is the position

In this role, you’ll use your clinical skills to make a real difference. You’ll start by reviewing referral files to see if an individual’s needs fit Resilience Healthcare Services. Then, you’ll hit the road, visiting people in their homes and day centres, or other settings, to understand how their disability affects their daily lives and how the services offered by Resilience Healthcare might be of benefit.

A dedicated scheduling team will arrange your visits, so you can focus on meeting clients, speaking with their families and carers, and gathering insights from professionals. Using a company car, you’ll travel between locations to build a full picture of each person’s needs.

Your observations and findings will then be written into clear reports, helping our Referral Committee make decisions about services for each referred person. Alongside the assessments, you’ll keep accurate records, follow important policies like GDPR and Safeguarding, and attend regular training to grow your own skills. You’ll also learn about Resilience Healthcare’s Models Of Care and make regular visits to the services where people receive support.

Location:  Flexible. This is a national role with hybrid/remote working, which means the successful candidate could be based anywhere in Ireland.

About Resilience

Resilience Healthcare is an Irish owned health and social care provider, founded in 2011. Through the provision of residential day and community based services, we support over 300 people with Autism, intellectual, physical and sensory disabilities and people with Prader Willi Syndrome.

Qualifications

  • Applicants can be qualified in any of the following disciplines: B.A. Social Care, R.N.I.D., Behaviour Support Specialist, Occupational Therapist or Speech & Language Therapist.
  • Applicants must be registered with their relevant professional body (NMBI, CORU, BCBA, etc).

Key Attributes

Candidates must:

  • Have at least 3 years’ experience working in a similar area.
  • Be energetic and enjoy working in a fast-paced environment.
  • Be competent in gathering and screening information including clinical information, have excellent observational and assessment skills and robust report-writing ability.
  • Be proactive, keen to learn and receptive to feedback.
  • Possess excellent oral and written communication skills.
  • Have the ability to work autonomously and I.T competency is a must-have.
  • Be willing to travel extensively within Ireland, with a flexible approach to meet the business needs.

Roles and Responsibilities

  • Carry out initial desk-top assessment of a referred file in order to ascertain if the individuals’ diagnosis and needs meet the criteria for Resilience Healthcare Services.
  • Work closely with the Business Unit within Resilience Healthcare who schedules and coordinates your assessment visits.
  • Using your clinical experience to work with individuals who have a diverse range of conditions, to gain a clear understanding of how their disability, presentation and needs affect their activities of daily living and how these impact their daily life.
  • Interpret clinical information from relevant referral reports and gather your findings into a comprehensive report, alongside your observations of the person across their environments and from relevant family members and professionals.
  • Have a good understanding of Resilience Healthcare models of care and consider these throughout the assessment process, ensuring the individual’s needs are identified in accordance with these.
  • Ensure the information in the report is succinct and comprehensive to support the decision-making process of the Referrals, Admissions, Transfers and Discharge Committee in determining eligibility and suitability for Resilience Healthcare Services.
  • Be responsible for compliance with all relevant GDPR Legislation.
  • To ensure the appropriate maintenance of clinical and administrative records and support appropriate reporting arrangements.
  • To ensure implementation and adherence to established policies and procedures e.g. health and safety, infection control, Child Protection, Safeguarding etc.
  • Attend mandatory training and other relevant training as deemed appropriate by your line manager.
  • Visit and understand the needs of Resilience services that people will be referred into.

  • Assessment Tools and Techniques: Familiarity with assessment tools and techniques specific to social care settings
  • Confidentiality and Ethical Practice: Maintaining confidentiality and adhering to ethical guidelines.
  • Collaboration and Teamwork: Working collaboratively with other professionals, agencies, and stakeholders.
  • Documentation and Record-Keeping: Accurate and detailed documentation of assessment findings, decisions, strong record-keeping skills.
  • The ideal candidate will have knowledge and experience working within disability residential or day services, providing exceptional care and support to individuals with disabilities.
